Go to the home page and then read the first box, click the bottom that says "Contact Us". Then make your form in the comment box of the contact form.
You have to sign up with Weebly or sign in with Google, Facebook, Twitter, etc. -
Brownie usualy when i fill in a mod form i know what to put in there cos it says for example things like age:

Why should you be a mod:
If your a mod how can you help to improve the site: -
There really isn't a "moderator" form but in the contact us comment box just say what job you want and why you want it. :P
