Im thinking it could go back to his sister. He thinks at the ball "My sister would of loved going to one of these..." And then he recalls her actually wanting to go to prom and stuff but she couldn't because of the scars and bruises she had from her stepdad. Then that thought strikes him like lightning and then he goes on a thought tangent about how he gets to experience this and she couldn't. How he gets to live in a paradise free of evil and have a nice life in Dream World while his siblings are suffering in the real world. Then he gets a psychotic thought where he thinks that if he knew he would end up here after killing himself, what if he killed his siblings too? THEN he starts to hate himself for actually contemplating taking the lives of his brother and sister
