Question #3 should provide the option, "yes, one or more close relatives.
Questio n # 6 should provide "no" as an option
Question #10 Asks you to choose "All Ages" or "Young AND/or Older which if using the AND option makes one of the answers redundant.
Questio n #13 should include the option, "I can lie but I choose not to."
